PPO (Preferred Provider Organization) vision insurance is a type of insurance plan that provides coverage for various vision-related expenses. With a PPO vision insurance plan, you are allowed to visit any eye care professional you choose, including optometrists and ophthalmologists, without needing a referral. However, you may receive additional benefits or discounts when you visit a provider within the PPO network.

Plan Highlights

  • Eye Exams: Eye health exam, dilation, prescription, and refraction for glasses: Covered in full after a $10 copay, when going in-network.

  • Lenses: Single vision, lined bifocal, lined trifocal, lenticular: Covered in full after $25 eyewear copay, when going in-network.

  • Frames: The plan has a $150 allowance toward to cost of frames, when going in-network. An additional 20% discount may be available. Check with your in-network provider for this discount. The additional 20% discount is not available at Sam’s Club or Walmart. Effective September 1, 2024, frames will be covered once every 12 months.

  • Contact Lenses: Use your vision plan to get contact lenses instead of eyeglasses. When seeking treatment with an in-network provider, the plan includes a $150 elective contact lens allowance. If contact lenses are medically necessary, they are covered in full. Contact lens fittings are covered at no cost. Additional discounts may be available. Ask your in-network provider.
