PPO (Preferred Provider Organization) vision insurance is a type of insurance plan that provides coverage for various vision-related expenses. With a PPO vision insurance plan, you are allowed to visit any eye care professional you choose, including optometrists and ophthalmologists, without needing a referral. However, you may receive additional benefits or discounts when you visit a provider within the PPO network.

Plan Highlights
Eye Exams: Eye health exam, dilation, prescription, and refraction for glasses: Covered in full after a $10 copay, when going in-network.
Lenses: Single vision, lined bifocal, lined trifocal, lenticular: Covered in full after $25 eyewear copay, when going in-network.
Frames: The plan has a $150 allowance toward to cost of frames, when going in-network. An additional 20% discount may be available. Check with your in-network provider for this discount. The additional 20% discount is not available at Sam’s Club or Walmart. Effective September 1, 2024, frames will be covered once every 12 months.
Contact Lenses: Use your vision plan to get contact lenses instead of eyeglasses. When seeking treatment with an in-network provider, the plan includes a $150 elective contact lens allowance. If contact lenses are medically necessary, they are covered in full. Additional discounts may be available. Ask your in-network provider.
